Certificate of unfitness for vaccination and notification
19.—(1) If any medical practitioner is of the opinion that any child is not in a fit and proper state to be vaccinated against diphtheria or measles or has any medical condition for which such vaccination is contraindicated, he shall forthwith issue to the parent or guardian of the child a certificate of unfitness for vaccination in the form set out in the Fourth Schedule.
(2) Where a child is temporarily unfit for vaccination, the vaccination shall be postponed to a date specified in the certificate of unfitness, which shall be a date not later than 3 months from the date of the certificate.
(3) Before the expiry of any such certificate, the parent or guardian shall take or cause the child to be taken to a medical practitioner, who shall then examine the child and either vaccinate the child or give another certificate of unfitness for vaccination.
(4) Any medical practitioner who issues a certificate of unfitness under this regulation shall within 7 days thereof notify the Director in such form as the Director may approve
