Notification of vaccination
3.—(1)  For the purposes of section 48(2)(b)(i) of the Act, every medical practitioner who carries out, or supervises the carrying out of, a COVID‑19 vaccination must, within 72 hours after the carrying out of the vaccination, notify the Director or an officer of the Board designated by the Director, in writing, of all of the following information in relation to the vaccination:
(a)the name and identification document number of the person vaccinated;
(b)the name of the manufacturer and batch number of the vaccine used;
(c)the date the vaccination was carried out.
(2)  For the purposes of section 48(2)(b)(ii) of the Act, every medical practitioner who carries out, or supervises the carrying out of, a COVID‑19 vaccination must, within 72 hours after the carrying out of the vaccination, notify the person vaccinated, in writing, of all of the following information in relation to the vaccination:
(a)the name of the manufacturer of the vaccine used;
(b)whether the vaccine administered was the first or second dose of the vaccine;
(c)the date and place the vaccination was carried out.
