2.  The First Schedule to the Parliamentary Elections Act is amended by deleting Forms 1, 8, 9, 9A, 10 and 12 and substituting the following Forms, respectively:

Whereas by section 24(1) of the Parliamentary Elections Act, it is provided that for the purposes of every general election of Members of Parliament, and for the purposes of the election of Members to supply vacancies caused by death, resignation or otherwise, the President shall issue writs under the public seal, addressed to the Returning Officer:
1And Whereas I think it is expedient that writs should be issued for the election of Members to serve in Parliament:
1  to be included in a writ for a general election.
2And Whereas the seat of the Elected Member for the Electoral Division of     has become vacant.
2  to be included in a writ for a by-election.
3 And Whereas the seats of the Elected Members for the Electoral Division of    have become vacant.
3  to be included in a writ for a by-election in a group representation constituency.
Now, therefore, I,        , the President, do hereby require that you, after giving notice of the time and place or places fixed for the nomination of candidates, cause election or elections to be made according to law for the Electoral Divisions specified in the Schedule to this Writ and that, if necessary, you do cause a poll or polls to be taken on the date required to be notified under section 34 (5) (b) or 34A(5)(b), as the case may be, of the Parliamentary Elections Act.
The places of nomination for the Electoral Divisions are given in the Schedule to this Writ.
