12.—(1) For the purposes of section 23(3) of the Act, a practising certificate shall be granted for a period not exceeding 5 years from the date of issue or renewal of the practising certificate.
(2) A registered allied health professional shall, at the time of an application for the grant or a renewal of a practising certificate, pay to the Council for the period that the practising certificate is to be valid, the practising fee specified in the Third Schedule.
(3) The holder of a practising certificate shall make available his original practising certificate when requested to do so in his practice as an allied health professional.
(4) Any person who fails to comply with paragraph (3) shall be guilty of an offence and shall be liable on conviction to a fine not exceeding $5,000.
