3.—(1) For the purposes of section 21(1)(a) of the Act, an application for registration as an allied health professional must be accompanied by the following:
(a)
unless dispensed with by the Council —
(i)
a certified true copy of the diploma or degree entitling the applicant to be registered;
(ii)
a transcript of the diploma or degree referred to in sub-paragraph (i) issued by the institution of higher learning that conferred the diploma or degree;
(iii)
a certified true copy of the applicant’s identity card or passport;
(iv)
a recent passport size photograph of the applicant;
(v)
the details of employment in Singapore or offer of employment to work in Singapore;
(vi)
where applicable, details in writing of the applicant’s professional practice experience in his prescribed allied health profession, including details of the nature, duration and extent of the experience in such form as the Council may require; and
(vii)
where the applicant is registered in his prescribed allied health profession in a foreign country, a certificate from the registration body of that country stating, as at the date of issue of the certificate —
(A)
that the applicant is a registered person in his prescribed allied health profession in that country;
(B)
the details of any complaint or disciplinary proceeding that has been taken or is pending against him in that country;
(C)
the specific details of any condition or undertaking that apply to the applicant’s registration in that country;
(D)
the specific details of any suspension or cancellation of the applicant’s registration in that country; and
(E)
any other information recorded in the register pertaining to the applicant’s registration in that country; and
(b)
any other information, statement or document (including any statutory declaration) required by the Council.
(2) Any document referred to in paragraph (1) which is not in English must be accompanied by a certified translation thereof in English.
