12.—(1) An application for the restoration of a name to the register shall be addressed to the Registrar.
(2) The application shall —
(a)
state the grounds on which it is made;
(b)
be in the Form 5 in the Schedule; and
(c)
be accompanied by at least two certificates of the applicant’s identity and good character in the Form 6 in the Schedule.
(3) The certificates may be signed by a registered pharmacist, a magistrate or a justice of the peace.
(4) On receipt of the application, the Registrar shall refer it to the Board for consideration.
(5) Except with the leave of the Board, neither the applicant nor any other person may appear before the Board to make any representation in respect of the application.
(6) The Board shall consider the application in private and may require the applicant to furnish such further documents or explanation as it thinks fit.
(7) If the Board directs the applicant’s name to be restored to the register, the Registrar shall inform him accordingly.
