Infectious Diseases Act
(Chapter 137)
Infectious Diseases (Prescribed Form) Regulations 2000
In exercise of the powers conferred by sections 6 and 72 of the Infectious Diseases Act, the Minister for Health hereby makes the following Regulations:
Citation and commencement
1.  These Regulations may be cited as the Infectious Diseases (Prescribed Form) Regulations 2000 and shall come into operation on 1st October 2000.
Form
2.  The form of notification of infectious diseases to be given under section 6 of the Act shall be in the form set out in the Schedule.
Revocation
3.  The Infectious Diseases (Prescribed Form) Regulations (Rg 4) are revoked.
